Providing urgent financial assistance to patients and their families since 1991

Social Worker Login

Thank you for visiting our website.

If you believe that you have patients that could benefit from financial support from us the following information will explain what we do, and how you can make applications to us.

We support children and adults who have had, or who are waiting for a solid organ transplant in the UK, by providing one off grants of up to £375 to purchase specific items, or help with additional costs associated with transplant.

We are unable to help with cost of living support, household bill or any debts.

All of our referrals for grant requests come directly from transplant professionals – usually social workers, but we can register any hospital professionals. Please contact us for more information. We normally have one referrer from each transplant centre to prevent cross referring.

We can only offer a one off grant – future requests for individuals will be declined.

Please note – we are unable to accept referrers from other charities that provide financial support.

Register with us –

Registering is easy – just send us an email and we will ask you for some information and get you set up.

Once you are registered you can then log into the ‘social worker log in’ portal and make grant referrals and check on the status of submitted requests.

We aim to make a decision on each request within 48 hours and, once agreed, we aim to make payment within 24 hours.

Making the grant application:

We always ask that you have ensured that patients are claiming all applicable benefits, and that there are no alternative funding sources before making an application to us.

When completing the application please be specific about the request, as non specific requests can delay the application.

For travelling expenses claims please ensure that the person is not entitled to help with health care costs, and let us know that this has been explored prior to the application.

Give us a brief overview of financial situation including any benefits claimed or applied for.

Examples of successful grant request are:

  • Urgent travelling expenses.
  • Urgent accommodation expenses.
  • A fridge to store essential medications.
  • Urgent costs associated with rehousing.
  • A cooker to feed a family.

Examples of grants that have been declined:

  • Financial help when benefits have been declined.
  • For taxis to clinic appointments.
  • To replace items in rented accommodation where this is the responsibility of the landlord set out in the tenancy agreement.
  • To pay back any money borrowed, or debts to utility companies.

The maximum grant application is £375, however, in certain circumstances we have approved a grant at a lesser amount, so pleas be mindful of this in your request and if possible only ask for the amount required.

This ensures that the grant approval is not delayed.

If you have any further questions, or are unsure if we can help, please contact us. We aim to respond to all emails within 24 hours.

We look forward to working with you and supporting your patients.
